• OFFICE HOURS - Monday - Friday, 8:30 AM - 5:00 PM (Early morning hours can be available through VIP Service)
• OFFICE VISITS - We make every attempt to avoid wasting your time with long waits. Your time is as valuable as ours.
• VIP SERVICE - Call our office manager at 943-0400 to inquire about benefits available through this special service.
• REGISTRATION - We will send you a patient questionnaire to fill out before your initial visit or you can download the patient registration forms from the Patient Section. This will enable us to provide you with appropriate care and keep you from a longer office visit. Please bring the form to your appointment. Your referring physician will be sent a report from us about your examination and treatment.
• PRIVACY - Rest assured that all information is strictly confidential and access to your medical records is limited to protect your privacy.
• EMERGENCY CARE - Dr. Cooper's patients may access an emergency service during off hours in order to contact Dr. Cooper. Leave a message and she will return your call as soon as possible at the number you leave or you should proceed to your nearest emergency room.
• INSURANCE - Dr. Cooper is a participating provider for Medicare and most insurance plans in the Greater Cincinnati area. Our insurance department automatically processes your claims. To do so, we need your signature or authorization on file, the name and address of your insurer and the name and I.D. number of the insured person. Please bring in your insurance card at the time of your initial visit and again if your plan should change.
• REFERRALS - We know that a referral is the highest compliment we can receive. We are committed to giving you the best possible care and take pride in our professionalism and the team effort of our expertly trained staff. New patients are always welcome. If your insurance company requires a referral, please make sure our office receives the referral information prior to your appointment.
• CANCELLATIONS - Please be kind enough to notify us 24 hours in advance so we can accommodate the schedules of other patients.
